College Prep Education - Early Alert Referral Form

The Central Florida Community College (CFCC), College Prep Education - Early Alert Referral Program was developed to assist in the overall effort to identify students not making satisfactory progress and/or exhibiting behaviors that may lead to academic difficulties. As part of our collaborative retention efforts, this on-line referral form was developed that links instructional faculty with the College Prep Education Office. Once a student is identified, the Enrollment Services Office and/or appropriate Student Support Services personnel will contact and work with the student in an effort to increase their chances of becoming academically successful at CFCC.
